Hypertension
I10 - Essential (primary) htn
Includes: high b/p
HTN with Heart Disease
Heart conditions (i50.- or I51.4-I51.9) are assigned a code from I11-Hypertensive heart disease when a causal relationship is
stated as due to hen or implied as hypertensive.
--Use add'l code from I50 to identify the type of heart failure
--If the causal relationship btw hen and heart disease is not documented, each condition is coded separately
HTN with CKD
Code from I12-Hypertensive chronic kidney disease when conditions classified to N18-CKD are present. Unlike HTN with heart disease, ICD-10-CM presumes a cause and effect
relationship and classifies renal failure with HTN as Hypertensive Renal Disease.
--Code from N18-CKD should be reported to identify the stage of the CKD, as a secondary dx
--Hypertensive CKD AND acute renal failure - use add'l code for the ARF.
Hypertensive Heart and CKD
Combination codes I13-Hypertensive heart and CKD
--Assume a relationship btw the HTN and CKD.
--Assign add'l code from I50 for
type of heart failure
--Code from N18 is used to identify the stage of CKD
--Hypertensive CKD and ARF, use add'l code for ARF.
Hypertensive Cerebrovascular Disease
I60-I69 - Cerebrovascular Disease
+ code for the hypertension
Hypertensive Retinopathy
Increased pressure leads to growth of unstable vessels of the eye.
These vessels can fracture and bleed, causing complications to vision and eye circulation.
Two codes are necessary:
--H35.0 - Background retinopathy and retinal vascular changes
--I10-I15 - indicate the type of HTN.
HTN, secondary
Endocrine disorders, CNS disorders, Vascular problems, medications can all cause HTN.
Requires two codes:
--underlying etiology
--I15 for secondary HTN
Reason for
the encounter determines sequencing
HTN, transient
R03.0 - Elevated B/P reading w/o diagnosis of HTN
O13.- or O14.- for transient HTN of pregnancy
Coronary Artery Disease
a blockage of the coronary arteries
Most common type is atherosclerosis
--I25 - Chronic ischemic heart disease
--Add'l code for exposure to or use
of tobacco
--Add'l code to report the areas of atherosclerosis
CABG
code is selected based on type of graft.
Autologous, biological - graft taken from patient
Non-autologous biological
Atherosclerotic heart disease with angina
a causal relationship can be assumed, unless documentation indicates the angina is due to
something other than the atherosclerosis.
These are combo codes.
Cerebrovascular Accident
Stroke or CVA w/ no add'l info - I63.9-Cerebral infarction, unspecified
Sequela of CVA
I69 - conditions classifiable to I60-I67 as causes of sequela
Sequela includes neurological deficits that persist after initial onset of the CVA.
These may be present from the onset or may arise at any time after the onset.

Pine Valley Bank has three branches. You have been assigned to evaluate the efficiency of each. The following inputs and outputs are to be used for the study. Input 1 = labor hours used (hundreds per month) Input 2 = space used (in hundreds of square feet) Input 3 = supplies used per month (in dollars) Output 1 = loan applications per month Output 2 = deposits processed per month (in thousands) Output 3 = checks processed per month (in thousands) The relevant information is given in Table 48. Use this data to determine if any bank branches are inefficient. If any bank branches are inefficient, determine the nature of the inefficiency. Table 48: $$ \begin{matrix} \text{ } & \text{Inputs} & \text{ } & \text{ } & \text{Outputs}\\ \text{Bank} & \text{1} & \text{2} & \text{3} & \text{1} & \text{2} & \text{3}\\ \text{1} & \text{15} & \text{20} & \text{50} & \text{200} & \text{15} & \text{35}\\ \text{2} & \text{14} & \text{23} & \text{51} & \text{220} & \text{18} & \text{45}\\ \text{3} & \text{16} & \text{19} & \text{51} & \text{210} & \text{17} & \text{20}\\ \end{matrix} $$

Suppose a, b, e, and dare positive integers . Prove each biconditional statement. (a) ae divides be if and only if a divides b. (b) a + 1 divides band b divides b + 3 if and only if a = 2 and b = 3. (c) a+ c = b and 2b - a= d if and only if a= b - e and b + e = d. (d) a+ 2e $\ne$d or b - a $\ne$2d if and only if b + 2e $\ne$ 3d or 3a + 4e $\ne$ b. (e) $a^2$, a+ b, and a+ b + e are all odd if and only if ab+ be and b + e are even and a+ e is odd.

Newcor’s steel mill has received an order for 25 tons of steel. The steel must be 5% carbon and 5% molybdenum by weight. The steel is manufactured by combining three types of metal: steel ingots, scrap steel, and alloys. Font steel ingots are available for purchase. The weight (in tons), cost per ton, carbon and molybdenum content of each ingot are given in Table 97. Three types of alloys can be purchased. The cost per ton and chemical makeup of each alloy are given in Table 98. Steel scrap may be purchased at a cost of $100 per ton. Steel scrap contains 3% carbon and 9% molybdenum. Formulate a mixed integer programming problem whose solution will tell Newcor how to minimize the cost of filling their order. TABLE 97: $$ \begin{matrix} \text{Ingot} & \text{Weight} & \text{Cost per Ton (\$)} & \text{Carbon \\%} & \text{Molybdenum \\%}\\ \text{1} & \text{5} & \text{350} & \text{5} & \text{3}\\ \text{2} & \text{3} & \text{330} & \text{4} & \text{3}\\ \text{3} & \text{4} & \text{310} & \text{5} & \text{4}\\\text{4} & \text{6} & \text{280} & \text{3} & \text{4}\\ \end{matrix} $$ TABLE 98: $$ \begin{matrix} \text{Alloy} & \text{Cost per Ton (\$)} & \text{Carbon \\%} & \text{Molybdenum \\%}\\ \text{1} & \text{500} & \text{8} & \text{6}\\ \text{2} & \text{450} & \text{7} & \text{7}\\ \text{3} & \text{400} & \text{6} & \text{ }\\ \end{matrix} $$
